Getting into the Program
Our staff works with various organizations to coordinate treatment procedures and payment options and supply “return to work” authorizations by a SAMHSA accredited physician.
Documents
- Verification of date of birth and current address
- Social Security card
- If not a citizen, a green card or green card number
- Private health insurance or Medicaid card
Our Benefits Division staff will work with incoming clients to access any additional documentation that may be missing.
All adolescents without a High School diploma attend school while at Daytop. Accordingly, in addition to other intake documents, students must have:
- Most recent school transcript
- Last school report card
- Current class schedule
- Most recent IEP (Individual Education Plan) − if applicable
- Immunization records
All school documents can be obtained from the last school attended by the student. If a parent does not have a copy of the Immunization records, the school of residence usually has the most recent.
Fees
Daytop accepts fee for service, some private insurance, and Medicaid. If potential clients are deemed eligible for government funding, we will assist with the application process.
